Since 1979, Community Hospice has been the Compassionate GuideSM for advanced illness management in Northeast Florida, helping people live better with a chronic condition. Community Hospice is the state’s only hospice provider, among the top 2 percent nationally, to earn Hospice Honors Elite Status two years in a row, a designation based on family evaluations.
In addition to providing comprehensive, high-quality hospice care, the organization offers several programs that help improve quality of life through relief of suffering in our community, such as Community PedsCare®, a nationally recognized pediatric palliative and hospice program for children with life-threatening and life-limiting conditions; Community Palliative Consultants, a consultative service for people with serious and complex illnesses that helps them make important health care decisions; and Community Care Choices, a CMS research model to help people with advanced illness find greater access to comfort care and support.
The Community Hospice Neviaser Educational Institute provides education and other resources for health care professionals and the community about dealing with serious illnesses and their impact. The Community Hospice FoundationSM generates philanthropic and community support for patient care services, family needs and community programs, supplemented with contributions raised by the Community Hospice Thrift Shops (www.thriftshopjax.com).
