Community PedsCare is a national-award-winning* pediatric palliative and hospice program for children with life-threatening conditions. A program of Community Hospice of Northeast Florida in collaboration with Wolfson Children’s Hospital, Nemours Children’s Specialty Care and the University of Florida — Jacksonville, Community PedsCare offers support, comfort and care to these children and their families.
We focus on the holistic, unique needs of children and their families and the value of a specialized program of support and resources. The Community PedsCare team customizes its care plan to meet the needs and abilities of each child. The program offers in-hospital and in-home medical, nursing, psychosocial, spiritual, volunteer and other services to support, comfort and care for children and their families.
Children with a progressive illness or sudden injury have a multitude of needs, and their families are often left facing difficult decisions and ongoing stresses. Community PedsCare brings comfort and care to enhance quality of life for patients from prenatal (before birth) to age 21 in the five-county Northeast Florida area … supporting children’s parents, siblings, other family, caregivers, classmates and teachers.
*In 2009, Community PedsCare won the National Quality in Palliative Care Leadership Award. The award is presented annually to recognize organizations that have enhanced their palliative care services by implementing the palliative care principles supported by the National Consensus Project for Quality Palliative Care and the National Quality Forum.
