I have always believed that when you change a death, you change a life. Since 1979, Community Hospice of Northeast Florida has been changing lives, one patient and family at a time.

We believe that quality of life is just as important as the quantity of life. Our goal is to provide each patient and family with care that alleviates pain and suffering, whether physical, emotional or spiritual, so that whatever time remaining is the best it can be. That focus on total care and quality time extends not only to the patient, but to family members and loved ones who also need support for a difficult time. 

Wherever patients and families need this care and support, the professionals who make up our interdisciplinary teams are grateful for the privilege to be there with them and to help them live better. As we touch their lives, they inspire us and remind us that the work we do every day truly matters. 

Thanks to their confidence in our care and the support of many others in our community, we have grown from a small and determined group of volunteers into a hospice provider that has touched the lives of tens of thousands of Northeast Florida residents. 

And as we have grown, the community has grown—expecting us to be there to provide care, to be a compassionate guide, and to serve as a partner to our local medical and long term care providers. We are grateful for the many patients, families, healthcare professionals, donors and volunteers who believe in our work and join us in fulfilling our mission of “quality of life care.” 

We believe that having a full-service, nonprofit hospice dedicated to providing care to all who need it improves the quality of life for everyone in our area. Through our work, we hope to touch many more lives for years to come. 

It has been our honor to be the community’s hospice of choice for these past three decades, and we look forward to continuing to be Northeast Florida’s compassionate guide for those who live with advanced illness.
