Community PedsCare Pediatric Program

Community PedsCare is a nationally recognized pediatric program for children in Northeast and North Central Florida living with complex, chronic, life-limiting or advanced illnesses. The Community PedsCare team supports the specialized pediatric needs and medical care of children from the prenatal stage to age 21. With a family-centered care approach, the Community PedsCare team enhances the lives of families through compassion and holistic care.

Why Community PedsCare?

The Community PedsCare Program Provides children and their families with a continuum of care that extends beyond the physician's office and goes with the child through their care journey.
  • Additional Pediatric & Family Support When Needed
    • Complementary care services, including integrative/alternative therapies, Comfort Touch, music therapy and pet therapy
    • Child life services, supporting siblings with therapeutic play
    • Child/family advocacy with education and health care systems
    • Therapeutic support and social group activities embracing children and families
    • Seamless support through care settings and transitions from pediatric to adult care systems
    • Respite care, offering parents and guardians a break from caregiving

    • Medical care, nursing care, and home nursing services

Who is Eligible For This Program?

Children may be referred by health and other child-serving professionals and organizations, clergy, family members and legal guardians. All referrals must be approved in consultation with the child's primary care physician and accepted by the caregiver.

How Are These Services Paid For?

Most Community PedsCare services are provided without charge regardless of insurance coverage. All vital pediatric care and support is provided at no cost to the family which is made possible through generous community supporters, efforts of Community Hospice & Palliative Care Foundation and Medicare.
  • Children Are Not Denied Access

    Charitable gifts to Community Hospice & Palliative Care Foundation provide vital funding for this care. Generous community support makes it possible for Community PedsCare to provide seamless care from the outpatient to inpatient setting, whenever and wherever families need it most.
