Community Care Choices

If you’re a Northeast Florida resident living with advanced illness, you may qualify for a new kind of care that offers the comfort and support of hospice, without giving up hope for a cure.

Community Care Choices is part of a Centers for Medicare & Medicaid Services (CMS) research effort. It’s for people who qualify for hospice care but wish to continue curative treatments.

How Can Community Care Choices Help?

Our compassionate care specialists, including licensed clinical staff and other health professionals, work with you, your loved ones and your physician to provide select hospice supportive services in your home. We help you:

  • Get some of the benefits of hospice care without giving up curative treatments.
  • Get the support you need, through case management and care coordination, to live better at home without additional office visits.
  • Maintain control of your health care by getting the information you need to make informed choices.
  • Know what to expect and better understand options for future care.

These services are available by phone 24 hours a day, anytime you need help. And for those times when you need someone in person, we come to you.

While we help you think through your options and support your decisions, we never make them for you. You and your family’s needs always come first, and you’re always in control.

Is it Right for Me?

Community Care Choices is for people certified by their physician with an advanced illness and meet hospice eligibility requirements, including limited life expectancy. However, it is not hospice care.

If you meet eligibility requirements for hospice care and the following criteria, you may be a candidate for Community Care Choices:

  • Enrollment in Medicare Parts A and B; not enrolled in Medicare managed care for at least two years
  • Diagnosis of advanced cancer, congestive heart failure, chronic obstructive pulmonary disease or HIV/AIDS
  • At least one hospital encounter (i.e. ED visit, observation stay or admission) in the last 12 months for any diagnosis or symptom
  • Three office visits with a Medicare-approved provider related to the qualifying diagnosis within the past year
  • Meets hospice eligibility and admission criteria
  • Has not elected Medicare/Medicaid hospice benefits within 30 days
  • Lives in a traditional home (excludes skilled nursing, assisted living, hospice inpatient unit or other institutional settings)

Even if you don't meet these criteria, our staff can provide you with information about other care options to help you live better, including palliative and hospice support.

Is There a Cost?

For Medicare beneficiaries who qualify for Community Care Choices, there is no out-of-pocket cost for the select hospice services.

Through the Community Care Choices Model, the Centers for Medicare & Medicaid Services (CMS) offers a new option for Medicare beneficiaries to receive select hospice services from certain hospice providers while concurrently receiving curative services from their primary care providers.
