Honoring Our Veterans
We provide specialized, dignified, compassionate care for our veteran patients. Our physicians, nurses, and social workers are trained to identify the end-of-life needs of veterans, and ensure they are honored for their service and sacrifices. They are also trained to recognize and address the emotional, moral, and spiritual pain that can result from combat. Helping them with these visible and invisible wounds of war can be especially important as a veteran nears the end of their life.

Today, and every day, we honor veterans.
Every veteran patient is asked about their time in the military from the day of admission and throughout their care journey. Some patients may choose to share their stories while others may not, but our team is trained to support the unique physical and psychological needs of veterans and their families. We believe it is our mission, just as our veterans accepted their mission to serve our country, to compassionately guide our patients and their families when facing end-of-life care decisions.
Most of our care is provided in the comfort of our patient’s homes, but there are times when 24/7 care for acute symptoms is needed. Veterans in need of around-the-clock care can be admitted to one of our centers for caring. While there, a branch of service flag will be hung outside their room to recognize their choice to serve our great nation.
Trained clinicians identify and address the psychological, physical, spiritual and emotional needs of veterans and their caregivers.
We provide a veteran-to-veteran pinning ceremony to honor the service of our nation’s bravest.
For veterans who receive care at our inpatient centers, we fly the flag of their service branch outside their room and entrust the flag to the family following death.
A select group of veteran leaders serving as brand ambassadors throughout Northeast and North Central Florida bringing awareness to, and providing access for, veterans and their caregivers facing advanced illness and end-of-life care.

Hospice Care
Hospice care is appropriate for people who have received a prognosis of 12-months or less to live. (6-months for Medicare)

Palliative Care
If you or someone you love lives with a serious or complex illness, palliative care can help, regardless of your age or the stage of the illness.

Caregiver Support
If you or someone you know has experienced the death of a loved one, the support of others can play an invaluable role in the healing process.
