When is it time?

Hospice care is appropriate for individuals with a prognosis of six months or less to live, as determined by their doctor (12 months or less for others). Many families share that they wish they had reached out sooner. Hospice services focus on improving comfort, maintaining mobility, and supporting meaningful social activities, helping patients remain engaged and in control of their lives for as long as possible.

If you or your loved one is not yet ready for hospice care, we also offer Palliative Care, which can be provided alongside curative treatments. Our Palliative Care program, available to Medicare beneficiaries, works closely with you and your physician to manage symptoms and improve the quality of life for those with serious illness.

Will I Have Control?

We give you the freedom to decide how you want to live your life and choose the kind of care and treatments you desire to receive. We create an individualized care plan centered around your needs and listening to input from your family and physician. You and your family’s needs always come first, and you’re always in control.

Is Hospice Care Covered by Insurance?

Medicare, Medicaid, and most private insurance cover most hospice care services. As a nonprofit organization, we also offer financial assistance to ensure that no one is turned away based on their ability to pay.

Must I Have Cancer?

We provide hospice care for all patients regardless of terminal illnesses, including congestive heart failure, stroke, chronic obstructive pulmonary disease, Alzheimer’s disease, end-stage dementia, kidney disease, liver disease, Parkinson’s disease, Lou Gehrig’s disease, complications of AIDS, multiple sclerosis, cancer, and other illnesses.

Do I Have to Stop Other Treatments?

You do not have to discontinue treatments like chemotherapy or physical therapy if they provide comfort. Each patient’s needs are unique, and we tailor our care to suit individual situations.

What Happens If I Can’t Communicate?

Our Advance Care Planning services ensure your wishes are documented and honored, even if you cannot speak for yourself. Through the Five Wishes program, we help you create a personal plan that covers your medical, emotional, and spiritual care preferences, ensuring your voice is heard in critical moments.
