Community Hospice & Palliative Care understands caring for a seriously ill loved one can be challenging. The whole family feels the effects of advanced illness. Families often need as much, if not more, attention as the patient. Our care supports the entire family, with guidance to help you make the most of the time that remains. We will manage pain, help your loved one feel comfortable, and help you understand what to expect.
Advance care planning (ACP) is a process of understanding, reflecting on and discussing your experiences, values, and beliefs in an effort to identify how you want to live if faced with a serious or life-limiting illness. It may include identifying the care you would want at the end of your life. It is important to choose someone to speak on your behalf if you are unable to make your own decisions and take comfort in knowing that person will convey your wishes. You can also decide if you want to complete an ACP document, often referred to as an advance directive. Finally, you will want to communicate your wishes to your family, friends, clergy, other advisors, physicians, and other health care professionals to help ensure that your wishes are honored. We offer both the Five Wishes and Honoring Choices advance care planning forms at no charge, and if you call us, trained staff are available to answer your questions and provide free assistance.
Palliative care is specialized medical support that improves quality of life for those with serious illnesses. Our team collaborates with you, your loved ones, and your doctor to create a personalized plan that eases physical, emotional, and spiritual pain. We explain your options and honor your wishes.
Unlike hospice, palliative care is available at any stage of illness and can be provided alongside curative treatments, working with—not replacing—your primary care physician.

Hospice Care
Hospice care is appropriate for people who have received a prognosis of 12-months or less to live. (6-months for Medicare)
