Providing the Support You Need
At Community Hospice & Palliative Care, we prioritize personalized care plans tailored to your unique needs and preferences, collaborating closely with your physician. Your individualized plan typically includes:
- Assessment of your and your family’s specific needs during your time with us.
- Identification of personal goals to guide your care journey.
- Detailed schedules outlining the frequency, types, and levels of services.
- Management of prescribed medications for symptom relief.
- Provision of necessary medical equipment and supplies.
- Specialized support for veterans.
- Certified Jewish hospice services accredited by the National Institute for Jewish Hospice, ensuring culturally sensitive care.
Our commitment is to provide comprehensive support that respects your values and enhances your quality of life.
Community Hospice & Palliative Care provides patients with medical supplies and durable medical equipment (DME) necessary for their care.
Items may include:
- Hospital beds
- Special mattresses
- Oxygen concentrators
- Portable oxygen
- Wheelchairs and walkers
- Nebulizers
- Over-bed tables
- Bedside commodes
- Shower chairs
Our DME staff is available to provide:
- Equipment setup, installation, maintenance and pickup
- Replenishment of medical supplies, as needed
- Safety requirements, and instructions for use and maintenance of equipment
- Assistance for medical equipment questions and concerns
We provide medications for treatment of symptoms related to the hospice diagnosis and in accordance with the patient’s individualized plan of care.
Community Hospice & Palliative Care physicians and pharmacists are experts in pain and symptom control, and review all medications with the medical director as part of a comprehensive assessment to ensure the best symptom management possible.
Medication Management for Patients Residing at Home
Destruction of Patient Medications in the Home, Long Term Care or Assisted Living Facility
After admission, we may issue a “comfort care kit” containing emergency medications that you can use in a time of crisis. This sealed package contains several medications needed on an urgent basis when symptoms such as constipation, pain, anxiety, nausea or vomiting occur.
Contact the Community Hospice & Palliative Care 24/7 Patient Priority Line to have a nurse review your situation and, upon an order from our physician, may instruct you to open and use a medication from the kit.
On a quarterly basis, Medicare patients will receive from the Center for Medicare & Medicaid Services (CMS) a Medicare Summary Notice that lists Medicare health insurance claims information and all the services and supplies billed to Medicare for a 90-day-period—for both hospice and non-hospice claims.
The Medicare Summary Notice is not a bill. Hospice care is an “all-inclusive” benefit; you do not owe any money for any of the hospice care provided to you by Community Hospice & Palliative Care. Hospice care providers are reimbursed a daily, or “per diem,” charge for the care, supplies, medical equipment and prescription medications provided to you that are related to your hospice diagnosis.
Medicare requires hospice providers to supply detailed information on all claims submitted for reimbursement. This includes every visit made by our nurses, hospice aides, physicians and psychosocial staff each month. In addition, Medicare also requires that the hospice assign a “charge” for each of these visits. These visit amounts appear on the Medicare Summary Notice as “noncovered charges.”
Note: To date, Medicare has not asked hospice providers to report visit details for other members of the hospice care team, such as chaplains, volunteers, bereavement specialists and others, even though these services may be provided to you.
The additional “non-covered charges” are for reporting purposes only, and are not added to, or in addition to, the per diem reimbursement received from the government by Community Hospice & Palliative Care.
You are not responsible for any difference between the non-covered charges and Medicare’s per diem reimbursement to Community Hospice & Palliative Care. Community Hospice & Palliative Care understands that your Medicare Summary Notice may raise questions. Again, the notice is not a bill, and you do not owe any money for the hospice care being provided to you.
