Every day we honor veterans throughout Northeast and North Central Florida by providing specialized, dignified, compassionate care to honor their service and sacrifices.
We have cared for and honored veterans from all branches of service since our founding in 1979.
Last year, we cared for nearly 1,800 veterans in 16 counties throughout Northeast and North Central Florida, with half utilizing one of our ten centers for caring to help get their symptoms under control and give their loved ones a welcome respite. When walking through one of our centers, you will often see a branch of service flag outside a patient’s room. The flag is meant to honor and recognize the veteran patient in our care for their willingness to raise their hand in defense of our great nation.
We provide specialized, dignified, compassionate care for our veteran patients. Our physicians, nurses, and social workers are trained to identify the end-of-life needs of veterans, and ensure they are honored for their service and sacrifices. They are also trained to recognize and address the emotional, moral, and spiritual pain that can result from combat. Helping them with these visible and invisible wounds of war can be especially important as a veteran nears the end of their life.
Community Hospice & Palliative Care is a Level Three Partner of the We Honor Veterans program, a collaborative effort by the National Hospice & Palliative Care Organization (NHPCO) and the Department of Veterans Affairs (VA) to honor veterans receiving hospice care.
